Welcome to our clinic! We ask that new patients please take the time to read through our web site to learn a little bit about us. We recommend that you start with Are We A Good Fit For You? It will take you through some of our primary policies and procedures.

We also recommend that you download the New Patient Paperwork Packet. Please fill it out and bring it to your first office visit along with your insurance card and driver's license.

Please note that at your first appointment a parent must be present with the patient.
