You are the most important person on the health care team. Patient-centered means that you and your health care are the focus of your medical home team.
As part of our commitment to provide your child with the highest quality of care, we partner with you and collaborate with your child's other healthcare providers. 
PATIENT CENTERED MEDICAL HOME
A Patient-Centered Medical Home (PCMH) is a system of care in which a team of dedicated health professionals works together with you to provide the best posible care and health outcomes for your child. This means our practice, as Your Medical Home, is the central resource for all of your child's healthcare needs. 
YOUR ROLE AS A PARENT
  • Notify us of changes to your contact information. 
  • Keep us up-to-date on your child's medications, immunizations, allergies, conditions, tests, hopitalizations, and visits to other providers.
  • Inform us of any changes to your child's medical history.
  • Authorize your child's other providers to coordinate care with us.
  • Participate in your child's treatment plan and medical decision making.
  • Follow treatment plans and take medications as prescribed
  • Communicate with us, ask questions, and provide us feedback to help us improve our care for you.

  • Return calls and communicate results with you in a timely manner

COMPREHENSIVE PEDATRIC CARE
We provide comprehensive pediatric care from birth through high school graduation
  • Same day sick appointments
  • Acute care for illness and injuries
  • Preventative well child visits, screenings, and vaccinations
  • Referrals to pediatric specialists when indicated
  • Coordination of all specialties, including mental health
  • Evidence-based care according to the American Academy of Pediatrics(AAP) / Bright Furtures Guidelines
CHILDHOOD VACCINATIONS
We believe in vaccinating in accordance with the schedule outlined by the Centers for Disease Control and Prevention and the AAP.
