Eastern Shore Children’s Clinic was established in 1976 in Fairhope, Alabama. Many residents of the Eastern Shore have "grown up" with us and continue the tradition by choosing us as the pediatricians for their family.

School Medication, Health and Sport Physical Forms
Parents and students, please have as much information already completed on these forms as you possibly can prior to leaving the form for your physician to complete or before your appointment. This includes all past medical history, allergies, medications, and even demographic information that you may know. It will save everyone time and additional phone calls if all of the requested information is readily available on the form for the physician prior to their review and completion. Also, please be advised that these forms may take 24-48 hours to complete or longer if your physician is out of the office, so please get the forms to us well before their return deadline.
Flu Vaccine Clinic
It's that time again-Flu season is here! Don't get caught without your flu vaccine! We will be holding walk-in flu vaccination clinics for our patients every Tuesday and Thursday from 2-4pm beginning September 20, while supplies last.
Vaccine News:
Parents, remember that all students 11 yrs. of age and older, who are entering the 6th grade in Alabama schools this 2011-12 school year, must have a tetanus-diptheria-acellular pertussis (Tdap) vaccine. Feel free to contact our office if you are in doubt as to which immunizations your child may need. For more information on vaccines, please visit http://www.adph. org/immunization.
New Patients
New patients or patients that have had immunizations at another facility must bring their immunization record with them to their checkup appointment. We can not always get this information once you are in the office and this could result in having to be rescheduled for a later appointment. New patients must be accompanied by a biological parent or bring custody/adoption papers, social security card, insurance information and drivers license.
Cancellations should be made no later than 4 hours prior to your scheduled appointment time. Appointments not cancelled at least 4 hours prior will be considered "no shows". Arriving more than 30 minutes late for an appointment will also be considered a no-show. We reserve the right to bill $25.00 for each no show appointment. Excessive no shows may result in dismissal from the practice.
